In the ICU, yes, our flowsheets are paper-based.

Our flowsheets are crazy cool...all computerized and suck the data right off the monitors so you just click to pull it through.

I feel silly saying this, since I know EMRs are efficient, reduce errors, etc. (and I've voiced my support for them on many occasions), but one good thing about paper is that it forces a certain level of engagement. To chart on paper, you have to at least process the numbers, and I think that may be better for really assimilating the information about your patients. Just scanning in data, while it may be more accurate, may not provide the same mental engagement and recognition of a patient's current condition. Still, the pluses of an EMR probably outweigh the minuses.
